Questionnaire for the epidemiological screening for students participating in on-site teaching activities

George Emil Palade University of Medicine, Pharmacy, Science, and Technology of Targu Mures informs the students that in order to participate in each on-site teaching activity it is mandatory to complete the QUESTIONNAIRE FOR THE EPIDEMIOLOGICAL SCREENING, which can be downloaded HERE.

Completing the questionnaire is done before the start of the on-site teaching activities, once a day, regardless of the number of teaching activities in which the student participates.

Students will answer the questions in the questionnaire and will mention the faculty where they study, the year of study, the group, the teaching activity in which they will participate on the date of completion, name and surname. The document will be saved with the name and surname of the student, and will be sent to the e-mail address: chestionarepidemiologic@umfst.ro.
